INSURANCE INFORMATION

Most insurance companies will cover Medical Nutrition Therapy with a registered dietitian based on medical conditions. Please see below for more information on how to find out your coverage. WE DO NOT ACCEPT NEW PPO/EPO PATIENTS.


WHERE DO I BEGIN?

A referral from your doctor is required. Please call your doctor and request a referral with progress notes, labs, and medication list to (626) 774-7988

  • If you have an HMO and the physician association is Optum, Regal Medical Group, Lakeside Medical Group, Healthcare LA, Imperial Healthcare, Prospect Medical IPA, St. Vincents IPA or HealthSmart please contact your primary care physician and request for a doctors referral and provide JETSET DIETITIAN (Julie Ching, MS, RDN, CDE) as your preferred dietitian preference. Once your referral is approved we will call to schedule your initial assessment.

  • All patients are required to keep a credit card on file upon booking in the case of:

    1. Insurance denials for ANY reason (including no referral on file)
    2: Missed or canceled appointments - Missed or canceled appointments without 24-hour notice will be charged the full fee at the time of the appointment.
    3: Deductibles
    4: Co-insurances
    5: Partially paid claims

If coverage is not verified or if Julie is out-of-network then payment is due at the time of service, she is happy to assist you with reimbursements for nutrition appointments by submitting a claim to your insurance provider and providing a superbill. Health Savings Account (HSA) credit/debit cards are accepted for payment in addition to other major credit cards and cash.
